Assisted Living

If you are looking for assistance with day-to-day tasks, OAK CREST offers two levels of Assisted Living care. We provide quality, individualized care in comfortably designed homes amid beautifully landscaped grounds. Furnished rooms with private baths and your own personalized décor combine to make your space feel like just like home. A choice of floor plans, including an apartment or suite, can accommodate a couple or a single person who desires additional space.

Our staff of RNs, LPNs, Medical Assistants and Resident Assistants provide a safe home-like environment, allowing each resident to achieve his or her highest level of independence. The services we offer are individualized to meet resident’s needs through a professional care plan process which ensures clinical care of the highest quality.

The services included in Assisted Living are: three nutritious home-cooked meals, medication administration, assistance with personal care needs, opportunities for spiritual growth, a wide range of social activities, transportation to local shopping, housekeeping, laundry and linen service, utilities and cable TV.

Respite care is also available to those needing short-term stays during post-rehabilitation/hospitalization or for temporary relief from care giving responsibilities. Respite care is based on bed availability in each level of care on each campus. Those needing respite care may come for a weekend or an extended period of time.

Assisted Living Residents who require a one-person assist with Activities of Daily Living (ADLs) and transfers can be accommodated in this level of care. Residents experiencing the beginning stages of Alzheimer’s Disease, dementia or confusion may also be accommodated at this level. Residents must be able to feed themselves and be mostly continent.

Assisted Living Plus Residents who require up to a two-person assist with ADLs and transfers can be accommodated at this level. Staffing is provided to assist those who need help with feeding. Residents who experience incontinence and those in more advanced stages of Alzheimer’s Disease, dementia or confusion can have their needs met at this level. Assisted Living Plus provides a secure and safe setting.
